Chronic Disease Rehabilitation

Post-acute Phase (Recovery Period) Rehabilitation
Post-acute phase generally refers to day 4 to 21 days after an injury. Post-acute phase rehabilitation is designed to help patients return to normal life activities as soon as they’re ready. Before sending patients home, they are fully evaluated to ensure that they can safely resume daily living activities such as communication and walking without limitations.
